Participant eligibility requirements

You must be at least 18 years old.

You must live in Davis, Salt Lake, Tooele, Weber or Utah County.

You must apply and be approved to participate (see application steps below). Purchases made before you get approved are not eligible.

You must agree to the Terms and Conditions of the program.

How it worksStep 1: Submit a quick eligibility application

Fill out a quick 15-minute eligibility application to ensure you qualify for the program.

We’ll let you know if you are approved.

We need to know:

Your contact information (name, email, phone number)

Your age and current home address (with documentation verification uploaded)

If you qualify for the higher voucher amount based on your income (with documentation uploaded)
