Authorities report the passenger car driver died on scene. The driver of the cargo van was taken to the hospital with a possible foot or ankle injury.

A driver was traveling westbound in the eastbound lanes, Utah Highway Patrol Lt. Zach Randall said. "Sounds like one confirmed fatal and possibly a second," Randall continued.
