Clinician-scientists at the University of Utah’s John A. Moran Eye Center have identified which types of endophthalmitis, a rare but serious bacterial eye infection, pose the greatest danger to vision. The findings indicate that the specific type of bacteria causing the infection significantly influences whether patients recover vision or suffer serious complications.
Study methodology and findings
A research team including Christopher Conrady, MD, PhD, Akbar Shakoor, MD, and Albert T. Vitale, MD, analyzed more than 240 cases of endophthalmitis treated between 2012 and 2022 across four academic medical centers. The study, published in the American Journal of Ophthalmology, found that patients infected with more aggressive bacteria, such as certain Streptococcus and Enterococcus species, were much more likely to experience severe vision loss compared to those with common surface bacteria.
Implications for clinical treatment
While ophthalmologists have historically based treatment decisions on a patient's visual acuity at diagnosis, these findings suggest that understanding the specific organism involved is a critical factor. This knowledge could allow doctors to make faster, individualized decisions regarding when patients require surgery or antibiotic injections to clear an infection.

Christopher Conrady, an infectious eye disease specialist who led the research, stated that not all infections behave the same way and emphasized the need to identify dangerous infections faster to protect patient vision.
