Implementation and Placement Instructions
To implement the program, drivers must complete a Yellow Dot form and keep it in their vehicle's glove box. A corresponding Yellow Dot sticker should be placed on the inside top-left corner of the windshield, near the oil change sticker.
Required Setup Materials
Setting up the program requires three specific items: a completed Yellow Dot form, a photo of the driver for easy identification without hats or sunglasses, and a Yellow Dot sticker. The form should include all current medications and medical conditions.

Forms and stickers can be obtained through several Utah locations. These include the Utah Highway Safety Office, University of Utah Health, and Salt Lake County Library Locations.
