A preliminary investigation determined that McCormack was traveling southbound on SR-305 in a white Toyota Tundra. The driver allowed the pickup truck to travel off the roadway and onto the right dirt shoulder. After inputting steering to the left, the Toyota rotated and re-entered the southbound travel lane. The vehicle then crossed the centerline and the driver steered to the right, causing the Toyota to rotate and skid across the northbound travel lane before going off the roadway onto the dirt shoulder and overturning.

McCormack was not wearing a seatbelt at the time of the crash.
